We also need your support and invite you to assist us in our quest for excellence by participating in school activities, attending parent-teacher conferences and monitoring your child’s homework on a daily basis. Camden County Schools Online Student Enrollment Information
We would like to welcome you and your child to Camden County Schools! Please read through the information below to identify the documents required for enrollment. Complete all forms within this online packet in order to ensure a prompt enrollment for your child. ENROLLMENT WILL NOT BE COMPLETED UNTIL ALL DOCUMENTATION IS UPLOADED VIA THE FAMILY DASHBOARD.
- Completed Enrollment Packet
- Student Certified Birth Certificate
- Not a copy or the Mother's Record
- Student's Current Shot Record
- Proof of Residency
- One from the following list
- Copy of homeowner's deed or mortgage
- Copy of the lease/rental agreement, current rent receipt, or a notarized letter from the property affirming the rental
- A notarized letter estimating the date of occupancy from the builder if your residence is under construction but is not complete. Occupancy must be ensured within 90 days from the date of the letter.
- AND two from the following list
- Current power bill
- Current water bill
- Current utility bill
- Valid NC Driver's License with current Camden address
- NC Health Assessment Transmittal Form
- Must be completed on the state-required form available HERE.
Additional documentation that may be required:
- Copy of Custody Agreement
- If the birth parents that are named on the birth certificate do not reside at the same address
- If there is no custody agreement, the other parent must provide a notarized statement confirming he/she is aware that this student will be enrolled in Camden County Schools
- Guardianship Documentation
- If the party enrolling the student is not the biological parent
- Must be signed by a judge
- Shared Housing
- Must have owner of the property present deed or proof of mortgage, utility bills and ID
- If shared housing is a rental agreement, both parties must have their names on the lease which indicates that the owner is aware that multiple families are residing at the residence

Kindergarten Students
An up-to-date immunization record meeting the state requirements as described below and current health assessment/physical form must be provided to the school nurse within thirty (30) calendar days of first day of attendance
- Health Assessment Form
- Immunization Requirements
- MMR and Varicella - 2 doses of each
- Polio - 4th dose on/after the 4th birthday
- Diphtheria, Tetanus, and Pertussis (DTaP or Tdap - 5 doses, if the 4th dose was not given on/after the 4th birthday)
- Hepatitis B - 3 doses
- Haemophilus influenze type b (Hib) - 4 doses
- The 30-day rule described below applies

New Students to North Carolina
If you are enrolling a student that has never attended a public school in North Carolina, the following items are required:
- A completed health appraisal/physical must be submitted to the school within thirty (30) calendar days of attendance
- If a physical has been done one (1) year prior to the first day of school attendance, you may submit that
- If a physical has not been done within a calendar year prior to the first day of attendance, one must be completed on the state-required form available HERE.
- The 30-day rule described below applies
Student's going into 7th grade for the first time:
- Two (2) immunizations are required
- Meningococcal
- Tdap

Student's going into 12th grade for the first time:
- Two (2) immunizations are required
- Meningococcal

**The 30-Day Rule Is North Carolina Law**
- The parent/guardian is responsible for submitting all required documents (listed above) to the school within 30 calendar days of school attendance
- Documents must be submitted by September 21st
- Students who do not meet the requirements are not allowed to attend school
- Students will be re-admitted/allowed to return to school when documents are received
